Other Info: Happily married man. After a discussion with my wife we agreed that this is something we wanted to do. It's my personal belief that everyone regardless of sexual orientation or male infertility should be able to have children.

My wife and I have discussed all the methods of donations and are ok with whatever your preference is. I will not participate in any form of donation without proper screening.

Contact with the child - This is completely up to you and you're significant other. If you're decision is to remain in contact with the child I would rather do so as a "friend of the family" type situation until the child is old enough or parents decide it's time.

Contact with parents - I have a clean family history but I would at the very least maintain contact with the parents in case of medical necessity.

Legal donor contract - No donation of any type will be made without a signed and notarized document.
